NOTE:

Please indicate what type of record(s) you are requesting. Examples include: Incident/Accident report, Body Worn Camera (Presumptive redaction includes at minimum, a medium blur), or if other please specify.

Complete the following fields to help process your request as quickly as possibly:

  • Case or Event #
  • Date and Location of Incident

If you do not have the Case or Event # please provide as much supplemental information as possible.
